How do I know the blockage is in the lateral and not inside the house?

If every fixture on the lowest level is slow or backing up, the problem is downstream of the house. A single slow sink is not the same thing. A camera down the cleanout settles it without guesswork.

Where is the cleanout usually located?

Typically outside near the foundation on the sewer side of the house, sometimes at the property edge. Older places around here occasionally have none, which is worth knowing before there's a backup.
